What is Well Check ? Well Check is an ‘assessment’ and ‘signposting’ service offered by Age UK Bromsgrove & District to local residents aged 50 and over. We know that, as you get older, it is important that you keep as well and as independent as possible. We also know that some people need help in identifying which services may be able to help them do that. This is what our Well Check Service offers. How does Well Check work ? Well Check offers you an ’assessment’ or check, on the sort of things that you may be starting to find difficult, or perhaps confusing. Once this has been completed you will be given a personalised ‘checklist’ of the services that may be able to help you and how to get in touch with them. If you find doing this difficult, and there is no one else who can help you, then our Well Check team may be able to assist you, or in some cases even do it on your behalf. Once the help has been organised one of our Well Check team will contact you again after six months, and then again a year later, to review what has happened and to see if you need further help. Who does the Well Check Assessment ? Age UK Bromsgrove & District has two ‘Well Check Officers’ plus an administrator who helps them, particularly with the six and twelve monthly reviews. Our Well Check Officers are experienced in assessing people’s needs and have a thorough knowledge of the services which may be able to help you. When you contact us, our Well Check Officers will need to ask some detailed questions about you and your home in order to enable us to make an accurate assessment of how best we can help you, but they will ensure that everything you tell them will be kept in confidence and that they only act in your best interests and with your agreement. What does Well Check cost ? Help from our Well Check Officers and the advice that they offer is always free. You might have to pay for some of the services that might be suggested to you, but it will always be your choice whether you use any of these services.
