This service is designed to help individual older people who are experiencing a degree of confusion, perhaps as a result of as a result of learning difficulties or deteriorating mental health, and who require additional support and assistance to allow them to continue living on their own in a home of their own choosing. Typically one of our Care Assistants works with the older person for 4-5 hours per week encouraging them to maintain every-day life skills and, for example, to plan their next weeks meals and the shopping necessary as a result and to maintain the hygiene of their living areas. There is a charge for this service which is currently £8.50 per hour. Referrals are made by Health or Social Care professionals.
