Age Concern throughout Herefordshire and Worcestershire

Wellcheck is an assessment and signposting service offered to people aged 50 years and over.   Its aim is to maximise independance and quality of life.  

Our Wellcheck Officer will do a home visit and discuss the issues the older person (and their family) may find difficult.   The Wellcheck Officer would then identify and suggest the appropriate services to address these issues.
